Job Description
Job Summary We are seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our team in. In this vital role, you will provide personalized, high-quality care to individuals in their homes, supporting their health, safety, and well-being. Your expertise will help clients maintain independence and improve their quality of life through attentive caregiving and medical assistance. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature Perform specialized care tasks such as catheter care, hospice support, dementia and Alzheimer's care, and end-of-life assistance Utilize Electronic Health Record (EHR) systems to document patient progress, medical records, and care activities efficiently and securely in accordance with HIPAA standards Support patients with memory care needs by implementing personalized routines and ensuring a safe environment Operate medical equipment including Hoyer lifts and other assistive devices to promote patient safety during transfers and mobility tasks Prepare nutritious meals, assist with feeding when necessary, and maintain cleanliness through light housekeeping duties such as laundry and cleaning to ensure a healthy living space Provide emotional support and companionship to foster a positive environment for clients with developmental disabilities or geriatrics Experience Prior experience working as a Home Health Aide or in assisted living facilities is highly preferred Demonstrated experience caring for geriatric patients or individuals with disabilities, including dementia or Alzheimer's disease Familiarity with home health agency operations, medical documentation, and home & community care practices Proven ability to handle up to 25 lbs safely using assistive devices like Hoyer lifts when necessary Strong understanding of home healthcare regulations and compliance standards such as HIPAA Join us if you're passionate about providing exceptional patient care in a supportive environment where your skills truly make a difference.
