The ideal candidate will possess a strong background in home healthcare, geriatric care, and patient support, demonstrating professionalism, empathy, and attention to detail. This role offers an opportunity to make a meaningful difference in clients' lives by delivering personalized, respectful, and comprehensive care within their home environments. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and document vital signs such as blood pressure, pulse, temperature, and respiration rates accurately using EMR (Electronic Medical Records) systems. Administer medications following prescribed schedules while maintaining strict adherence to home health care regulations and HIPAA confidentiality standards. Support clients with specialized needs such as catheter care, dementia or Alzheimer's care, memory care, and end-of-life assistance. Utilize patient transfer methods and equipment like Hoyer lifts to ensure safe mobility and transfers for clients with limited mobility or disabilities. Maintain detailed medical documentation and records of patient progress, medication administration, and care activities. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Communicate effectively with clients' families and healthcare teams to coordinate care plans and update on patient status. Skills Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or palliative care. Strong knowledge of medical terminology, vital signs monitoring, medication administration, and basic life support (BLS) including CPR. Familiarity with EMR/EHR systems for accurate documentation of patient information and medical records. Ability to perform patient personal care assistance with compassion while maintaining patient confidentiality per HIPAA guidelines. Experience working in assisted living facilities, nursing homes, or home health agencies is highly desirable. Competence in handling heavy lifting safely and using patient transfer equipment such as Hoyer lifts or other mobility aids. Excellent communication skills for client interaction, team collaboration, and family engagement in a respectful manner. Pay:
