This is a meaningful opportunity for someone who is committed to promoting dignity, independence, safety, and quality of life while providing attentive, person-centered support. The ideal candidate will be patient, respectful, detail-oriented, and comfortable assisting with daily activities while helping the individual participate safely and meaningfully in his home and community. Responsibilities Provide compassionate, one-on-one support with activities of daily living (ADLs), including personal care, grooming, dressing, toileting, and hygiene. Assist with meal preparation, feeding, hydration, and nutrition according to the individual's needs and care plan. Monitor and observe the individual's condition and overall well-being, including taking vital signs when required, and promptly report any changes or concerns. Assist with safe transfers, repositioning, and mobility using appropriate techniques and equipment, including a Hoyer lift when applicable. Administer and/or assist with medications in accordance with the individual's care plan, applicable regulations, and established procedures. Provide supportive care for cognitive, developmental, or neurological needs using appropriate communication, redirection, memory-care, and behavioral-support techniques. Encourage independence and participation in daily routines while providing assistance when needed. Support the individual in accessing and participating in community activities, appointments, recreational activities, social events, and other meaningful experiences. Assist with transportation to medical appointments, errands, community activities, and other destinations as required, ensuring the individual's safety and comfort. Provide companionship and encourage meaningful activities, social interaction, skill development, and community participation. Promote the individual's independence, self-advocacy, personal choice, and inclusion within the community. Respect the individual's privacy, dignity, preferences, rights, and personal choices at all times. Maintain a clean, safe, organized, and comfortable environment while performing routine housekeeping and sanitation tasks related to the individual's care and support. Maintain strict confidentiality and comply with HIPAA and all applicable privacy requirements. Communicate effectively with family members, healthcare professionals, and other members of the individual's support team regarding needs, progress, and concerns. Follow the individual's established care plan and document services provided, observations, and significant changes accurately and promptly. Participate in required and ongoing training, including CPR, first aid, safe transfers, medication procedures, and other training relevant to the individual's support needs. Respond calmly and appropriately to changes in the individual's condition or support needs and seek assistance when necessary. Perform other duties as needed to support the individual's health, safety, independence, personal goals, and quality of life. Requirements Prior experience providing one-on-one support in community-based services, private-home care, home health, assisted living, nursing homes, group homes, or similar settings is highly preferred. Experience supporting individuals with developmental disabilities, intellectual disabilities, autism, dementia, Alzheimer's disease, or other cognitive or neurological conditions is a plus. Knowledge of personal care, individual monitoring, medication assistance/administration, and care-plan implementation. Ability to safely assist with transfers, mobility, positioning, and manual handling, including the use of equipment such as a Hoyer lift when required. Current CPR and First Aid certification required. Strong communication and interpersonal skills, with the ability to interact respectfully with the individual, family members, and members of the support team. Ability to maintain confidentiality and follow HIPAA requirements. Dependable, punctual, patient, compassionate, and emotionally mature. Strong attention to detail and the ability to recognize and promptly communicate changes in the individual's condition or support needs. Ability to work independently while following established care plans, service guidelines, and safety procedures. Must be physically capable of performing the essential duties of the position, including assisting with transfers and mobility as required by the individual's support needs. A valid driver's license and reliable transportation may be required for transportation to appointments, community activities, and other outings. Willingness to complete additional training related to the individual's specific needs and community-based services.
