Direct Support Professional (Goup Mentor)

Function:

The Group Mentor plays a vital role in providing direct support and care to individuals with developmental disabilities (Members). This position is responsible for fostering a therapeutic and supportive environment , promoting independence, and ensuring individuals are integrated into their communities with dignity and respect. Group Mentors implement positive behavioral support strategies, assist with personal care needs, and facilitate daily activities to enhance the quality of life for those they serve.

Key Responsibilities:

Direct Support & Care:

Provide one-on-one supervision and personal care to individuals, ensuring their safety and well-being . Assist with daily living activities , including toileting, feeding, bathing, dressing, and personal hygiene . Implement and follow individualized behavior plans developed by the behavioral specialist, if applicable. Encourage and teach life skills , including meal preparation, household tasks, and social interaction. Support individuals in making independent choices while providing guidance when needed. Ensure that dignity, respect, and person-centered support are at the core of all interactions.

Community Integration & Transportation:

Accompany and transport individuals to community outings, recreational activities, and medical appointments. Foster community engagement by encouraging participation in social, educational, and employment-related activities. Act as a liaison between individuals and community resources , helping them build confidence and independence.

Behavioral Support & Supervision:

Implement positive behavior support strategies to promote appropriate behavior and reduce behavioral challenges. Use de-escalation techniques when necessary and follow crisis intervention protocols. Ensure all interactions and interventions align with the individual's Support plan (IHP) .

Documentation & Compliance:

Maintain accurate daily documentation of behaviors, progress, special incidents, and interventions. Complete medication administration records and ensure individuals receive prescribed medications as required. Ensure all documentation meets Department of Human Services (DHS) regulations and organizational policies. Attend and participate in team meetings, training sessions, and performance reviews .

Training & Professional Development:

Complete all DHS-mandated training and continue skill development through the College of Direct Support, the Elizabeth M. Boggs Center, and other available training programs . Stay updated on best practices in disability support, person-centered planning, and behavioral interventions .

Additional Responsibilities:

Actively participate in team discussions and contribute to the overall success of the program. Maintain professionalism, respect, and confidentiality at all times. Carry out any additional duties as assigned by the program manager or leadership team.

Qualifications & Requirements:

Must be at least 18 years old .

Education:

Minimum of a High School Diploma or equivalent . Prior experience working with individuals with developmental disabilities or behavioral challenges is preferred.

Physical Requirements:

Must be able to assist with lifting, transferring, bending, sitting, and stooping . Must be able to lift and carry up to 50 lbs .

Skills & Abilities:

Strong written and verbal communication skills . Ability to work effectively as a team member in a diverse and inclusive environment. Ability to build positive relationships with individuals, families, and colleagues. Knowledge of behavioral support techniques, crisis intervention, and positive reinforcement strategies . Must be able to cooperate with DHS staff and organizational leadership during inspections or investigations.
