Overview We are seeking a dedicated and compassionate Certified Home Health Aide to join our team. The ideal candidate will have a strong background in home healthcare, with experience assisting clients with daily activities, managing medical needs, and providing emotional support. This role offers an opportunity to make a meaningful difference in the lives of seniors and individuals with disabilities by delivering high-quality, respectful care in a home setting. Responsibilities Provide personal care assistance including bathing, dressing, grooming, and mobility support to clients. Assist with activities of daily living (ADLs) such as toileting, feeding, and medication reminders. Monitor and document patient health status, vital signs, and changes in condition Support clients with mobility and transfer methods, including the use of Hoyer lifts or other patient transfer equipment. Administer medications accurately following home healthcare regulations and Director of Nursing instructions. Maintain patient confidentiality and adhere to HIPAA guidelines while handling sensitive medical records and information. Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Provide companionship and emotional support to promote mental well-being, especially for clients with dementia or Alzheimer's disease. Follow safety protocols including first aid, CPR certification, and basic life support procedures to respond effectively in emergencies. Collaborate with healthcare teams to ensure comprehensive patient care tailored to individual needs. Skills Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or memory care. Strong knowledge of catheter care, vital signs monitoring, and medication reminders. Excellent patient observation skills to detect changes in condition or behavior promptly. Ability to perform transfers when necessary and operate assistive devices such as Hoyer lifts safely. Experience working within home health agency settings or providing live-in care is preferred. Exceptional communication skills for effective client interaction and team collaboration. Knowledge of home health care regulations and best practices for long-term care environments.
