In this vital role, you will provide compassionate, personalized care to clients in their homes, supporting their health, safety, and independence. Your energy and attention to detail will make a meaningful difference in the lives of individuals requiring assistance with daily activities, medical needs, and emotional support. This position offers flexible hours and the opportunity to work in a rewarding environment where your caregiving skills are truly valued. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs and recognizing changes in condition Support clients with memory care needs such as Alzheimer's disease and dementia, ensuring safety and comfort Use electronic health record (EHR) systems to accurately document care provided and clock in/out for all shifts. Implement patient transfer methods safely using equipment as needed such as Hoyer lifts or gait belts Maintain patient confidentiality in accordance with HIPAA regulations and home healthcare standards Skills Strong patient observation skills to identify changes in condition or behavior Experience working with individuals with developmental disabilities, geriatrics, or in assisted living settings Knowledge of medical terminology, medication administration, and basic life support (BLS) including CPR and first aid Familiarity with home health care regulations, hospice care, and end-of-life support practices Proficiency in using EMR/EHR systems for documentation; experience with home health agency operations preferred Ability to perform heavy lifting safely and assist with patient transfers using proper techniques and equipment Excellent communication skills for effective client interaction and teamwork within healthcare settings This role is ideal for compassionate caregivers eager to make a positive impact while advancing their professional skills.
