Job Description
Job Overview We are seeking a dedicated and compassionate Hourly Certified Home Health Aid to join our team in providing exceptional in-home care for individuals with diverse needs. As a vital member of our caregiving staff, you will deliver personalized support that promotes independence, safety, and well-being. This role offers an opportunity to make a meaningful difference in the lives of clients while working in a dynamic and supportive environment. The ideal candidate is energetic, detail-oriented, and committed to delivering high-quality patient care with professionalism and empathy. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility assistance using tools like Hoyer lifts when necessary. Monitor and observe patient conditions, including vital signs, medication adherence, and overall health status, documenting findings accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques. Support clients with assisted living needs by helping with meal preparation, light housekeeping, laundry, and maintaining a clean and safe environment. Administer medications following prescribed instructions while adhering to HIPAA regulations to ensure patient confidentiality. Perform catheter care, wound care, hospice support, and other medical procedures as trained and authorized. Communicate effectively with healthcare teams regarding patient progress and document medical records thoroughly to ensure continuity of care. Qualifications Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA). Proven experience working with individuals with disabilities or in long-term care settings such as nursing homes or assisted living facilities. Knowledge of medical terminology, CPR certification, and medication administration protocols. Experience working with EMR/EHR systems for accurate documentation of patient information. Ability to perform patient observation skills including monitoring vital signs and recognizing changes in condition. Familiarity with dementia care, Alzheimer's care, hospice support, catheter care, and memory care is highly desirable. Strong caregiving skills combined with excellent communication abilities and compassion for patients' needs.
