Job Description

The Day Habilitation Support Professional reports directly to the Program Director or assigned designee. Responsible for providing direct support assistance and resources for persons with Intellectual Developmental Disabilities. Does involve daily pickup and drop-off as needed. Must have a fun, can-do positive attitude.
Qualifications:
High School Diploma or equivalent Knowledge of CCW Waiver Procedures or minimum 1 year of experience working with persons with Intellectual Development Disabilities or in another health-related field Must be able to use a computer, WORD, Email, Excel, Zoom Good communication, writing, and computer skills Valid driver's license in good standing no more than 2-3pts (NO
PROVISIONAL LICENSES
) Ability to work with a team Good organizational skills
CPR/First Aid/AED Certification Salary:
$19.00-$21.00 per hour Yearly bonus, based on work performance Responsibilities include: Provide ongoing protective oversight to people attending the Day Habilitation Program. Responsible for implementing the Day Habilitation Plan for each person following the safeguards stated in their ISP. Complete daily activity logs accurately and timely for people supported. Enter information accurately and contemporaneously on each person's billing sheets and/or electronic system, on a daily basis to ensure regulatory compliance. Implement behavioral support plans written and ensure associated data collection. Successful training and demonstrated competency of DDD training provided on NJ College of Direct Support and program training. Provides training, assistance, and/or supervision to the individual(s) in the areas of daily living, personal care, financial management, community, social integration, medication administration, and/or job skills development. Establishes a therapeutic relationship with the individual receiving services to ensure effective guidance, support, and service delivery. Ensures objectives are met per the Individual Service Plan. Assist in maintaining a clean environment. Proper use and maintenance of equipment and appliances. Drive agency vehicles safely to transport individuals to and from the program, and to various volunteer sites/ community sites daily. Perform other duties as assigned.
