In this vital role, you will provide personalized care and support to individuals with developmental disabilities, aging needs, or health challenges. Your enthusiasm and commitment will help enhance the quality of life for those you serve, fostering independence, safety, and well-being in a nurturing environment. This position offers an exciting opportunity to make a meaningful difference every day while working in a supportive and dynamic setting. Duties Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor patient health by taking vital signs, observing for changes, and reporting findings accurately while maintaining HIPAA compliance Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed Support residents in participating in community activities, social outings, and recreational programs to promote engagement and independence Administer medications following care plans and document all care activities meticulously in accordance with HIPAA regulations Maintain a clean and safe environment by performing light cleaning duties within the resident's living space and common areas Follow established care plans for residents with dementia, Alzheimer's, autism, or other disabilities, ensuring personalized attention and safety at all times Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments preferred Knowledge of patient observation techniques and ability to monitor vital signs accurately Certification or training in CPR and First Aid required; additional certifications such as HIPAA training or medication administration are a plus Experience with dementia care, memory care, hospice care, or working with individuals with developmental disabilities highly desirable Ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or wheelchairs when necessary Strong communication skills to effectively collaborate with team members, residents, families, and healthcare professionals Compassionate attitude with a genuine desire to support individuals' independence and dignity Join us now to become part of a passionate team committed to providing exceptional care!
