Job Description
Job Summary We are seeking a dedicated Home Health Aide / Personal Care Assistant to join our homecare team and provide compassionate, personalized care to patients in their homes. This role is central to helping individuals — particularly the elderly — maintain independence, comfort, and dignity as they age in place. The ideal candidate brings a professional, attentive approach to daily care, health monitoring, and emotional support, and is comfortable working with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or end-of-life care. Responsibilities Assist patients, with particular attention to the needs of elderly clients, with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report changes to healthcare professionals promptly Support medication reminders in accordance with prescribed protocols, maintaining strict compliance with documentation standards Assist with patient transfers using appropriate equipment (e.g., Hoyer lifts, transfer belts) to ensure safety during mobility tasks Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation, particularly for elderly and homebound patients Maintain a clean, safe home environment by assisting with light housekeeping, laundry, and meal preparation Accurately document all patient interactions in compliance with home healthcare regulations Qualifications Prior experience as a Home Health Aide, Personal Care Assistant, or in assisted living, or elder care settings strongly preferred Experience with dementia care, developmental disabilities, or geriatric care is a plus Working knowledge of medical terminology, catheter care, and end-of-life care practices preferred Familiarity with home health agency documentation systems desirable Strong communication skills and the ability to work independently while collaborating effectively with clients, families, and the healthcare team We are proud to support caregivers with flexible schedules and a collaborative team environment.
