Job Description

Overview We are seeking a compassionate and dependable Medical Social Worker to join our home health team on a per diem basis. In this role, you will provide psychosocial assessments, emotional support, counseling, and community resource coordination to patients and families in the home setting. The Medical Social Worker works collaboratively with the interdisciplinary team to help address social, emotional, environmental, and financial factors that may impact patient health, safety, and overall well-being. Essential Duties and Responsibilities Assess the psychosocial status of patients in relation to their illness, living environment, and support system, and communicate findings to the Registered Nurse and interdisciplinary team Conduct social evaluations and develop appropriate intervention plans based on identified patient and family needs Provide supportive counseling, emotional support, and guidance to patients and caregivers Assist patients and families with accessing community resources, financial assistance programs, and other support services Serve as a liaison between patients, families/caregivers, community agencies, and healthcare providers Participate in care planning, interdisciplinary case conferences, and discharge planning as needed Educate patients and families regarding available community resources, long-term planning, and practical environmental needs Maintain collaborative relationships with agency personnel and external community resources to support continuity of care Maintain timely, accurate, and complete clinical documentation in the electronic medical record system Participate in staff meetings and in-service education programs as required Assist physicians and other healthcare team members in understanding social and emotional factors related to the patient's condition and care Perform other duties as assigned by the Clinical Director or Supervisor Qualifications Graduate of a Bachelor of Social Work (BSW) program accredited by the Council on Social Work Education; Master of Social Work (MSW) preferred Current Nevada social work license in good standing Minimum of one (1) year of healthcare experience required Home health experience preferred Strong verbal, written, organizational, and interpersonal communication skills Ability to work independently and manage assigned visits efficiently Current CPR certification required Valid driver's license, reliable transportation, and current automobile insurance in accordance with state and agency requirements required Additional Information This position requires travel to patient homes within the agency's service area. Flexibility, professionalism, compassion, and the ability to maintain patient confidentiality are essential to success in this role.
