Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes promptly Administer medications accurately following prescribed schedules and medical documentation protocols Support patients with mobility transfers using proper patient transfer methods and equipment like Hoyer lifts or gait belts Maintain detailed medical records and documentation within Electronic Health Record (EHR) systems while ensuring compliance with HIPAA regulations to protect patient confidentiality Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment in accordance with home healthcare regulations Provide emotional support and companionship to enhance patient well-being, especially for those with dementia or experiencing end-of-life care needs Qualifications Proven experience working with individuals with developmental disabilities, geriatrics, dementia care, or hospice environments is highly preferred Knowledge of medical terminology, catheter care, medication administration, and vital signs measurement Prior experience in assisted living facilities or nursing homes is advantageous; home health agency experience is a plus Familiarity with EMR/EHR systems for medical documentation and home health care assistance procedures Certifications such as First Aid, Basic Life Support (BLS), CPR are required; additional certifications in Alzheimer's or memory care are beneficial Ability to perform heavy lifting safely and use patient transfer equipment effectively Strong communication skills to interact professionally with patients, families, and healthcare teams while maintaining patient confidentiality under HIPAA guidelines Embark on a rewarding career where your caregiving skills foster independence and improve quality of life.
