The ideal candidate will possess a strong commitment to patient well-being, with experience in senior care, medical assistance, and working with individuals with varying forms of disease diagnose. As an HHA, you will play a vital role in supporting patients' daily activities, ensuring their safety, comfort, and dignity while maintaining compliance with healthcare standards and regulations.What you will be doing:
Completes Hospice Aide duties following established plan of care. Provide 24-hour continuous care for patient/families in their home, as needed. Reports all patient condition changes and/or uncontrolled symptoms to primary nurse, on-call nurse, TL or DPS immediately. Maintains adequate records of all patient/family care services, as per policy. Report all cases of abuse/neglect to supervisor, as per policy. Completes required competencies, infection control and ins-services. Assists with orientation of new Hospice Aides as requested by supervisor. Participates in community education activities which promote Hudson Valley Hospice and its philosophy. Notifies Hospice in the event of patient changes and uncontrolled symptoms.Skills we are looking for:
Requires flexibility to meet assigned tasks. Ability to effectively communicate verbally (in person and by phone) and in writing. Capability to work effectively and cooperatively as a team member. Understanding of hospice philosophy and the care we provide Ability to prioritize and handle multiple tasks efficiently. Reliability Bilingual is a plus!Work Environment:
Travel by car to patient's home, nursing home or facility setting; reliable transportation required. Weekend on call coverage All shift availability - flexibility to work crisis-care hours when needed. Uniform - Scrubs required. Ability to carry at least twenty-five pounds of equipment and/or supplies. Moderate to high risk of exposure to unsafe environments. Routine duties involve potential for direct exposure to blood or body fluids Valid NYS driver's license with reliable transportation Education & Experience we are looking for: New York State certified home health aide (HHA) with a minimum of 12 hours per year of in-service education since receiving certification. Experience in Hospice care philosophy preferred. Experience working in hospice or home care setting preferredHours:
Sunday-Thursday; 8:30am-4:30pm; 40 hours per week (all HHA's do provide one weekend day per month on call rotation)Pay:
