Job Description

Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide essential support and care to individuals in their homes. The ideal candidate will have experience in patient care and a strong understanding of the needs of those requiring assistance, including seniors and individuals with disabilities. This role is vital in ensuring the comfort, safety, and well-being of our clients while promoting their independence. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting. Provide companionship and emotional support to clients, fostering a positive environment. Monitor and document vital signs and changes in clients' health status. Administer medications as prescribed and ensure proper medication management. Assist with mobility using equipment such as Hoyer lifts when necessary. Perform light housekeeping duties including cleaning, laundry, and meal preparation to maintain a safe living environment. Support clients with Alzheimer's care, dementia care, and other memory-related conditions. Maintain accurate medical documentation in compliance with HIPAA regulations. Collaborate with healthcare professionals to provide comprehensive care tailored to each client's needs. Skills Proficient in patient observation and assessment skills. Experience in assisted living or long-term care settings is preferred. Familiarity with EMR/EHR systems for effective medical documentation. Knowledge of medical terminology and basic nursing skills including CPR and catheter care. Previous experience working with people with developmental disabilities is a plus. Strong communication skills to interact effectively with clients, families, and healthcare teams. Ability to perform cleaning tasks and meal preparation safely and efficiently. Compassionate caregiver who demonstrates patience and empathy towards clients' needs.
