Job Overview We are seeking a compassionate and dedicated Certified Home Health Aide (HHA/PCA) to provide exceptional in-home care to individuals with diverse needs. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs and recognizing changes in condition; accurately document findings using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications and perform basic medical procedures like catheter care and wound management following medical terminology guidelines and HIPAA regulations Support patients with memory care needs, including Alzheimer's disease and dementia, ensuring a safe environment through specialized techniques in memory care and hospice settings Operate medical equipment such as Hoyer lifts for safe patient transfers; assist with mobility using appropriate techniques and safety protocols Prepare nutritious meals tailored to dietary restrictions; assist with feeding when necessary while maintaining patient dignity Perform light housekeeping duties including cleaning, laundry, and maintaining a safe living environment to promote overall health and comfort Maintain detailed medical documentation of patient care activities, vital signs, medication administration, and other relevant records in compliance with healthcare standards Requirements Valid certification as a Home Health Aide (HHA) with current state licensing where applicable Proven experience in home health care, long-term care facilities, nursing homes, or assisted living environments Strong knowledge of medical terminology, medication administration, and patient observation techniques Experience working with individuals with disabilities or developmental disabilities; familiarity with Alzheimer's care and dementia-specific strategies is highly desirable Ability to operate EMR/EHR systems efficiently for accurate documentation of patient information Valid Work Authorization/Valid Photo ID/Signed Social Security card Pre Employment Physical, Drug screen, QuantiFERON, Rubella and Rubeola (Within the Past Year) Excellent communication skills and compassionate approach to patient care; ability to work independently and as part of a team Physical stamina to perform caregiving duties including lifting, transferring patients using Hoyer lifts, and standing for extended periods Join us in delivering compassionate home health services that truly make a difference!
