Job Description

Overview Join our compassionate team as a Home Health Aide and make a meaningful difference in the lives of individuals needing personalized care at home. In this vital role, you will provide essential support to patients with diverse needs, including those with chronic conditions, disabilities, or recovering from illness. Your dedication will help promote independence, comfort, and dignity while ensuring safety and well-being in a home setting. This position offers an opportunity to work closely with healthcare professionals and deliver high-quality patient care in a supportive environment. Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and transfer devices Monitor patient health by taking vital signs, observing symptoms, and reporting changes promptly to healthcare teams Administer medications and manage medical documentation in compliance with HIPAA regulations and EMR/EHR systems Support patients with specialized needs including dementia, Alzheimer's disease, memory care, hospice care, and working with individuals with developmental disabilities or disabilities Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for patients Assist with medical procedures like catheter care, medication administration, and wound management under supervision or as trained Maintain accurate medical records and documentation of patient care activities for continuity of care Experience Prior experience providing patient care in home health settings or assisted living facilities is highly preferred Experience working with seniors or in long-term care environments enhances your ability to meet diverse patient needs Familiarity with medical terminology, HIPAA compliance, and electronic health record (EHR) systems is essential Demonstrated ability to perform patient observation accurately and respond effectively to emergencies such as CPR certification or hospice situations Hands-on experience assisting with ADLs, medication management, and specialized dementia or memory care is a strong asset Knowledge of medical procedures including vital signs measurement and catheter care will support your success in this role Valid certification in CPR and first aid is required; additional certifications related to caregiving or nursing home experience are advantageous Contact Ramon at (845) 743-1575 or (253) 999-3616
