Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure comfort and dignity. Monitor patient health by observing vital signs, recognizing changes, and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient privacy. Provide specialized care such as Alzheimer's and dementia care, memory care, hospice support, and working with individuals with developmental disabilities or disabilities. Operate medical equipment including Hoyer lifts, catheters, and assist with medical documentation to ensure safe and effective care delivery. Perform household tasks like cleaning, laundry, meal preparation, and ensuring a safe environment that promotes health and well-being. Document all patient interactions thoroughly in medical records, ensuring compliance with healthcare standards and privacy laws. Requirements Proven experience in home health care, assisted living facilities, nursing homes, or long-term care environments. Strong knowledge of medical terminology, vital signs measurement, medication administration, and patient observation techniques. Certifications such as CPR (Cardiopulmonary Resuscitation), first aid training, and any state-specific caregiving credentials are required. Experience working with individuals with disabilities or special needs including Alzheimer's disease or dementia is highly preferred. Ability to operate EMR/EHR systems efficiently for accurate documentation; familiarity with hospice care protocols is a plus. Excellent communication skills combined with compassion and patience to provide empathetic patient care. Flexibility to work live-in shifts or part-time hours as needed; valid driver's license is often required for community-based visits.
