Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility to promote comfort and independence Monitor patient health by observing vital signs, managing medication administration, and reporting changes in condition promptly Support patients with medical procedures including catheter care, wound care, and basic life support techniques such as CPR and first aid when necessary Maintain accurate medical documentation using Electronic Health Record (EHR) systems while ensuring compliance with HIPAA regulations to protect patient confidentiality Facilitate safe patient transfers utilizing proper patient transfer methods and equipment like Hoyer lifts or gait belts Provide companionship and emotional support to patients, especially those with dementia or Alzheimer's disease, fostering a caring environment Assist with household tasks such as meal preparation, cleaning, laundry, and light housekeeping to ensure a safe and comfortable living space Skills Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or memory care services Strong knowledge of home health care regulations, medical terminology, and documentation standards Skilled in patient observation to detect changes in condition or behavior promptly Familiarity with EMR/EHR systems for efficient recordkeeping and medical documentation Ability to perform patient personal care assistance including assisting with ADLs and mobility support using proper techniques Excellent communication skills for effective client interaction and collaboration with healthcare teams Physical stamina for heavy lifting, long shifts, and assisting with patient transfer methods safely Certification in CPR and first aid; experience working within home health agencies or long-term care settings is highly desirable Embark on a rewarding career where your compassion meets your expertise—help improve lives one day at a time!
