Work Leads to Independence or wli is looking for motivated individuals to join our team. wli is a non-profit assisting adults with disabilities to get jobs in the community and has been in business for over 25 years.

Responsibilities include:

Assist in outings in the community with individuals served. Adult Day Support - non residential setting and seperate from the persons home In-Person in an integrated community setting Ensures a healthy and safe environment for individuals served. Assists in the development and implementation of Individual Plans Assists in all required documentation of services performed. Attend ISP meetings as assigned, and implement all sections of the ISP Provide transportation to consumers, as needed or assigned Produce daily progress notes for each consumer and communicate important information by all appropriate means Assist in the periodic review and update of consumer file Responsible for delivering all employment and habitation services towards the attainment of IP/ISP goals. Assist in the development and maintenance of task analysis and environmental analysis for each service site. Be responsible for the generation of a service schedule based on the needs of persons served and input from other staff. •Drug test and background check required for all positions

$16.00 - $17.00 per hour
