Job Description

Job Overview Join our compassionate healthcare team as a Home Health Aide, where your dedication and caring nature will make a meaningful difference in the lives of patients and their families. In this vital role, you will provide personalized in-home care, assisting individuals with their daily activities and health needs. Your energetic approach and attention to detail will ensure that clients receive the highest quality of care, promoting independence, comfort, and well-being in their own homes. This position offers a rewarding opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those recovering from illness or injury. Responsibilities Deliver attentive patient observation to monitor health status and report any changes promptly. Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary. Manage medication administration accurately following prescribed schedules and medical instructions. Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities and patient information securely in compliance with HIPAA regulations. Support clients with mobility aids such as Hoyer lifts and transfer devices to ensure safe movement. Conduct vital signs assessments including blood pressure, pulse, temperature, and respiration rates. Assist with catheter care, wound management, and other specialized medical procedures under supervision or as trained. Provide companionship and emotional support to reduce feelings of loneliness or confusion, especially for clients with dementia or Alzheimer's disease. Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and organizing personal spaces. Support hospice care patients by respecting their dignity and comfort preferences during end-of-life care. Document all care activities thoroughly in medical records and communicate effectively with healthcare teams for coordinated patient care. Adhere strictly to safety protocols including CPR certification requirements and infection control procedures. Qualifications Proven experience as a Home Health Aide or in a similar caregiving role within assisted living facilities, nursing homes, or home health settings. Strong knowledge of patient observation techniques and ability to recognize changes in health status. Familiarity with medical terminology, medication administration protocols, and documentation standards such as HIPAA compliance. Experience working with people with developmental disabilities or dementia is highly desirable. Certification in CPR is required; additional certifications such as hospice care training or catheter care are advantageous. Ability to operate medical equipment like Hoyer lifts safely and effectively. Excellent communication skills to interact empathetically with patients, families, and healthcare professionals. Capable of working independently while maintaining high standards of patient care in home environments. Valid driver's license preferred for travel between client locations; reliable transportation required.
