$21 Service AreaSchedule Cleveland / Northeast Ohio | Flexible scheduling available. What You'll Do Provide personal care assistance to hospice and home health patients in their homes including bathing, grooming, dressing, and hygiene Assist patients with ambulation, transfers, and mobility as directed by the care plan Support patients with light housekeeping, meal preparation, and errands as outlined in the plan of care Observe and report changes in patient condition, mood, or behavior to the supervising nurse or therapist Provide companionship and emotional support to patients and families Assist hospice patients and families with comfort-focused care needs Maintain accurate and timely documentation in the EMR within required timeframes Follow all infection control, safety, and agency policies during every visit What Sets VNA Apart AI-assisted documentation- spend your time with patients, not paperwork 4-star CMS agency with CHAP accreditation and a 124-year legacy in Northeast Ohio Mileage reimbursement at the IRS standard rate Paid trainingCEUs through Relias Limited weekend/holiday requirements
