$21 Service AreaSchedule Cleveland / Northeast Ohio | Flexible scheduling available. What You'll Do Provide personal care assistance to hospice and home health patients in their homes including bathing, grooming, dressing, and hygiene Assist patients with ambulation, transfers, and mobility as directed by the care plan Support patients with light housekeeping, meal preparation, and errands as outlined in the plan of care Observe and report changes in patient condition, mood, or behavior to the supervising nurse or therapist Provide companionship and emotional support to patients and families Assist hospice patients and families with comfort-focused care needs Maintain accurate and timely documentation in the EMR within required timeframes Follow all infection control, safety, and agency policies during every visit What Sets VNA Apart AI-assisted documentation

Requirements Must-Have:
Active STNA certification (Ohio) Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area Genuine compassion and comfort working with seriously ill, elderly, or end-of-life patients Ability to follow a plan of care and work under the supervision of a licensed nurse or therapist Preferred /
Nice-to-Have:
Prior home health or hospice aide experience Experience providing personal care in a home-based or community setting Familiarity with HCHB or similar home health/hospice EMR Comfort supporting patients and families through serious illness or end-of-life care
