In this vital role, you will provide compassionate, personalized care that promotes independence, safety, and well-being. Whether supporting residents in group homes, assisted living facilities, or community settings, your energy and commitment will foster a nurturing environment where clients thrive. Duties Assist individuals with activities of daily living (ADLs) such as bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Support residents with medication administration, vital signs monitoring, and adherence to care plans to promote health and safety. Observe patients closely for changes in condition through patient observation and monitoring techniques; document findings accurately while respecting HIPAA privacy regulations. Prepare nutritious meals and assist with meal planning and cooking to meet dietary needs and preferences. Facilitate patient transfer methods using equipment like Hoyer lifts and manual handling techniques to ensure safe mobility. Provide transportation assistance for appointments, errands, or community activities to foster independence and social engagement. Maintain a clean, safe environment by performing light cleaning duties and adhering to infection control protocols. Support individuals with neurodevelopmental disorders, dementia, Alzheimer's disease, or other cognitive impairments through specialized memory care strategies. Collaborate with healthcare professionals to update care plans and ensure consistent delivery of quality patient care. Engage in ongoing training related to CPR, first aid, hospice care, patient personal care assistance, and other relevant certifications. Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care environments such as nursing homes or assisted living facilities. Strong knowledge of patient monitoring techniques including vital signs assessment and medication administration. Familiarity with specialized care such as Alzheimer's care, dementia care, hospice support, and memory care strategies. Ability to perform patient transfer methods safely using equipment like Hoyer lifts; manual handling skills are essential. Excellent communication skills for effective client interaction and collaboration with team members and healthcare providers. Experience working within HIPAA guidelines to protect patient confidentiality at all times. Capable of heavy lifting when necessary; physically fit for tasks involving manual handling or assisting with ADLs. Knowledge of home health practices including home & community care experience; experience working with individuals with neurodevelopmental disorders is highly valued. Certifications in CPR and first aid are required; additional training in geriatrics or social work is a plus. Demonstrated patience, compassion, and adaptability when supporting individuals with diverse needs across various settings.
