In this vital role, you will provide essential support to patients in their homes or assisted living environments, promoting independence, comfort, and well-being. Your expertise in patient observation, medication administration, and assisting with activities of daily living (ADLs) will ensure each individual receives compassionate, respectful, and professional care. Duties Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor and document vital signs and patient condition changes accurately using Electronic Health Record (EHR) systems Support medication administration following medical terminology guidelines and HIPAA privacy standards Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory care needs, including behavioral management and safety supervision Assist with medical procedures such as catheter care, wound care, and hospice support as directed by healthcare professionals Prepare nutritious meals tailored to dietary restrictions and assist with feeding when needed; perform light housekeeping tasks including cleaning and laundry to maintain a safe environment Maintain detailed medical documentation and records in compliance with healthcare regulations while ensuring patient confidentiality Requirements Proven experience in home health care, nursing homes, assisted living facilities, or long-term care environments Knowledge of medical terminology, vital signs measurement, medication administration, and infection control practices Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus Experience working with individuals with developmental disabilities or disabilities requiring specialized attention Ability to operate EMR (Electronic Medical Records) systems and EHR (Electronic Health Record) systems efficiently Strong patient observation skills to identify changes in condition or behavior promptly Compassionate attitude with excellent communication skills to collaborate effectively with patients, families, and healthcare teams Flexibility for live-in employment options or scheduled shifts that meet patient needs We welcome dedicated professionals eager to provide exceptional home health services.
