This position offers an exciting opportunity to make a meaningful difference in patients' lives by assisting with daily activities, ensuring safety, and promoting independence. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that enhances well-being and supports recovery or ongoing health management. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, including vital signs and overall health status Support medication administration and manage medical documentation accurately using Electronic Health Record (EHR) systems Provide specialized care such as catheter care, dementia and Alzheimer's care, and end-of-life support Help with light housekeeping tasks like cleaning, laundry, meal preparation, and maintaining a safe environment Use patient transfer methods and equipment such as Hoyer lifts to ensure safe mobility and transfers Communicate effectively with clients and their families to address concerns and update on care progress while maintaining patient confidentiality in accordance with HIPAA regulations Requirements Proven experience working with individuals requiring home healthcare assistance, including senior care, hospice care, or clients with developmental disabilities or dementia Knowledge of medical terminology, vital signs monitoring, medication administration, and basic life support (BLS) or CPR certification Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts Experience working within home health care regulations and maintaining compliance with HIPAA standards Strong caregiving skills with a compassionate approach to patient personal care assistance and client communication Valid certification in first aid and CPR; additional certifications in hospice or dementia care are advantageous Prior experience in assisted living facilities or nursing homes is preferred; experience working with individuals with disabilities is highly valued Ability to work independently while following detailed care plans and protocols in a home setting Join us in delivering exceptional home health services that empower patients to live healthier, happier lives.
