Job Description

Job Overview Join our compassionate team as a Home Health Aide, where you will play a vital role in delivering personalized, high-quality care to individuals in their homes. This energetic position offers the opportunity to make a meaningful difference in patients' lives by assisting with daily activities, providing emotional support, and ensuring safety and comfort. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to promote independence and well-being in a warm, supportive environment. Responsibilities Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary. Monitor and observe patients' conditions regularly, recording vital signs and reporting any changes to healthcare providers promptly. Assist with medication administration following prescribed instructions while maintaining strict adherence to HIPAA regulations for patient privacy. Support patients with meal preparation and nutritional needs, ensuring dietary restrictions are followed accurately. Manage medical documentation and update electronic health record (EHR) or electronic medical record (EMR) systems efficiently to maintain accurate patient records. Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall health and safety. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions, including memory care techniques and behavioral support. Assist with catheter care, hospice care needs, and other complex medical procedures as trained. Work effectively within assisted living or long-term care settings, supporting residents' physical and emotional needs with patience and professionalism. Collaborate with healthcare teams to ensure comprehensive patient care plans are followed accurately. Experience Prior experience working in home health care or senior care environments is highly preferred. Experience with nursing home settings or long-term care facilities is advantageous. Familiarity with medical terminology, medical documentation practices, and EHR/EMR systems is essential. Demonstrated ability to work with individuals with disabilities or developmental disabilities is desirable. Certification in CPR and first aid is required; additional certifications such as hospice care training or dementia care are a plus. Proven ability to handle sensitive situations with compassion while maintaining patient confidentiality under HIPAA guidelines.
