Position Summary:

The Transition Direct Support Professional (DSP) provides temporary direct care and transition support to ensure individuals experience a smooth start to services and uninterrupted care. This position assists with new client admissions, Direct Support Professional orientation, initial service implementation, emergency staffing coverage, and continuity of care until a permanent Direct Support Professional is assigned. The Transition DSP promotes OneWell Health Care's commitment to person-centered, safe, and high-quality services.

Position Scope:

The Transition Direct Support Professional serves individuals receiving home and community-based services throughout assigned Ohio service areas. This position works closely with Operations, Clinical Services, Intake, Scheduling, Direct Support Professionals, individuals, families, and referral sources to ensure successful service implementation and ongoing continuity of care.

Key Responsibilities:

The Transition Direct Support Professional (DSP) is responsible for supporting new participant admissions, service implementation, and continuity of care during transitions and staffing shortages. Responsibilities include providing person-centered direct support, assisting with activities of daily living, community integration, and medication reminders as authorized; participating in admission visits and transition meetings; orienting newly assigned Direct Support Professionals to participant care plans and preferences; maintaining accurate and timely documentation; monitoring and reporting changes in participant condition; responding to emergency staffing needs; ensuring compliance with safety, infection control, and regulatory requirements; participating in training and quality improvement initiatives; and performing other duties as assigned.

Confidentiality:

Maintain strict confidentiality of all protected health information, employee information, and company business information in accordance with HIPAA and company policies.

Documentation Requirements:

Maintain accurate, timely, and complete documentation including: Service notes Care documentation Incident reports Required electronic documentation Communication logs Other records required by payer, regulatory agencies, or

OneWell Health Care Certifications:

Required:

CPR and First Aid certification (or ability to obtain within the required timeframe) All certifications required under Ohio regulations for assigned services

Education:

Required:

High School Diploma or GED Preferred:

Postsecondary education in healthcare, human services, psychology, nursing, or a related field.

Experience:

Required:

Experience providing direct care or direct support services.

Preferred:

Experience supporting individuals with developmental disabilities, intellectual disabilities, behavioral health needs, or physical disabilities. Experience in home care or community-based services.

Technical Skills:

Electronic documentation systems Mobile applications for scheduling and documentation Basic Microsoft Office skills Ability to use smartphones, tablets, and company technology Time management and organizational skills

Languages:

Must be able to read, write, and communicate effectively in English. Additional languages are considered an asset based on service area needs.

Driver License:

Required:

Valid driver's license. Reliable transportation. Ability to meet OneWell driving requirements. Ability to travel between service locations.

Travel Requirement:

Frequent travel throughout assigned Ohio service areas is required. This position requires flexibility to provide services in multiple locations and may require evening, weekend, holiday, and emergency coverage based on operational needs.
