In this vital role, you will provide essential support to individuals across various stages of life, including seniors, those with disabilities, or patients recovering from illness. Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, including vital signs and overall health status Administer medications and perform catheter care following medical instructions and protocols Support patients with memory care needs, dementia, Alzheimer's disease, and end-of-life care with patience and empathy Maintain accurate medical documentation using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring HIPAA compliance and confidentiality Facilitate safe patient transfers utilizing proper methods such as patient lifts or Hoyer lifts Assist with meal preparation, light housekeeping, laundry, and cleaning to promote a safe and comfortable environment Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and updates Follow home healthcare regulations and safety procedures during all caregiving activities Qualifications Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred Knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (BLS) or CPR certification required Experience working with individuals with developmental disabilities or geriatrics highly desirable Ability to perform heavy lifting safely and use patient transfer methods effectively Familiarity with EMR/EHR systems for medical documentation is a plus Strong communication skills with a compassionate approach to patient care Valid certification in first aid and CPR required; additional certifications in hospice or dementia care are advantageous Must adhere to home health care assistance standards while maintaining patient confidentiality under HIPAA regulations Join us as a Home Health Aide where your dedication fuels better health outcomes!
