Full Job Description Job Summary:

We are seeking compassionate individuals to join our team as Home Health Aides. As a Home Health Aide, you will provide essential care and support to individuals in their homes, assisting with daily living activities and ensuring their well-being.

Responsibilities:

• Assist clients with personal care tasks, including bathing, grooming, and toileting
• Prepare and serve nutritious meals according to dietary needs
• Provide assistance with mobility and physical therapy exercises
• Reminders and assist with medication management
• Perform light housekeeping tasks to maintain a clean and safe environment
• Monitor and report changes in client's health status to the appropriate healthcare professionals

Requirements:

• Experience in home care or a related field is preferred
• Ability to lift and transfer clients safely
• Knowledge of medical records systems and documentation procedures
• Proficiency in meal preparation, including specialized diets .
• Familiarity with memory care practices for clients with cognitive impairments
• Strong communication and interpersonal skills to provide compassionate care
