Job Description

Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; document findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Medication reminders following prescribed protocols while maintaining strict adherence to HIPAA regulations to ensure patient confidentiality Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients Maintain detailed medical documentation and records of patient care activities in compliance with healthcare standards Collaborate with healthcare teams to coordinate patient care plans and communicate changes or concerns effectively Experience Prior experience in home health care or assisted living environments preferred; experience in nursing homes or long-term care settings is highly valued Demonstrated ability to work with individuals with disabilities or complex medical conditions such as dementia or Alzheimer's disease Familiarity with medical terminology and medical documentation practices Certification in CPR and first aid required; additional certifications such as catheter care or hospice care are advantageous Experience operating EMR/EHR systems for accurate record keeping and communication Proven ability to handle sensitive situations with compassion while maintaining professionalism and confidentiality Strong understanding of patient observation techniques and medication administration protocols Join us to be part of a dedicated team committed to enriching lives through compassionate home health care.
