Home Health Aide (HHA)

Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient health status, including vital signs and overall condition, reporting any changes promptly Support medication administration following prescribed protocols while maintaining accurate medical documentation Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities and patient information securely in compliance with HIPAA regulations Operate medical equipment like Hoyer lifts and assist with transfers safely to prevent injury Prepare nutritious meals and assist with meal planning and preparation to meet individual dietary needs Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for the patient Support patients during community outings or errands as needed for home & community care Experience Prior experience working in assisted living facilities, nursing homes, or home health environments preferred Demonstrated experience caring for seniors or individuals with disabilities or chronic conditions such as Alzheimer's or dementia Familiarity with medical terminology and medical documentation practices required Certification in CPR is necessary; additional certifications such as hospice care training or medication administration are advantageous Experience operating EHR/EHR systems and documenting patient records accurately is highly valued Long-term care experience and working with individuals with developmental disabilities or memory care needs will strengthen your application Ability to work independently in a home setting while maintaining professional standards of patient care Join us to empower individuals through compassionate caregiving!
