Twin Maple Home Health is looking for HHAs for Jackson, McArthur an Hamden areas!! Job Summary Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. This position offers an enriching opportunity to support patients with diverse needs, including elderly seniors, individuals with disabilities, and those requiring specialized memory or hospice care. Your efforts will promote independence, comfort, and well-being while fostering trusting relationships with clients and their families. As a Home Health Aide, you will be an essential part of delivering high-quality home and community-based care that makes a meaningful difference every day. Responsibilities Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or wheelchairs. Monitor and observe patients' health status by recording vital signs and noting changes in condition or behavior. Administer medications and treatments following medical documentation protocols while ensuring compliance with HIPAA regulations for patient privacy. Support patients with memory care needs, including Alzheimer's and dementia care, creating a safe environment tailored to their cognitive challenges. Document all patient interactions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to maintain comprehensive medical records. Provide companionship and emotional support to enhance patients' mental well-being during home visits or live-in assignments. Assist with household tasks such as meal preparation, light cleaning, laundry, and ensuring a clean, safe living environment aligned with home & community care standards. Collaborate effectively with nurses, physicians, and other healthcare professionals to coordinate patient care plans and report any concerns promptly. Skills Proven experience in home health care or long-term care environments, including nursing homes or assisted living facilities. Strong knowledge of medical terminology, medication administration procedures, and vital sign measurement. Skilled in patient observation techniques to detect changes in condition or behavior promptly. Familiarity with EMR/EHR systems for accurate documentation of patient records and medical histories. Experience working with individuals with disabilities or developmental disabilities; capable of providing specialized dementia or Alzheimer's care. Ability to operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively. CPR certification is required; additional certifications such as hospice care training or hospice certification are advantageous. Excellent communication skills to interact empathetically with patients and family members while maintaining confidentiality under HIPAA guidelines. Capable of performing caregiving tasks including meal preparation, light housekeeping, cleaning, and laundry efficiently. Demonstrated reliability in home & community settings with a flexible approach to live-in or part-time employment arrangements.
