Job Description
Overview Join our compassionate healthcare team as a Home Health Aide, where your dedication and caring nature will make a meaningful difference in the lives of patients receiving in-home care. In this vital role, you will provide personalized assistance to individuals across various stages of life, including seniors, those with disabilities, and patients requiring end-of-life support. Your energy and empathy will help promote independence, comfort, and well-being in the familiar setting of their homes. Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs and reporting changes to healthcare professionals Administer medications and perform catheter care following medical instructions and safety protocols Support patients with memory care needs, including dementia and Alzheimer's disease, ensuring a safe environment Document patient care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining HIPAA confidentiality Help with meal preparation, light housekeeping, laundry, and cleaning to promote a clean and comfortable living space Use patient transfer methods such as Hoyer lifts or transfer belts safely to assist with mobility and prevent injury Requirements Proven experience working with individuals with disabilities, geriatrics, or in assisted living or nursing home settings Knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (BLS) including CPR and first aid certification Familiarity with home health care regulations and documentation standards for medical records and client communication Ability to work independently while maintaining excellent patient care and confidentiality under HIPAA guidelines Physical ability to perform heavy lifting, assist with patient transfers, and provide end-of-life or hospice care as needed Strong communication skills to effectively interact with patients, families, and healthcare teams Prior experience using EMR/EHR systems for medical documentation is preferred Join us in delivering exceptional home health care assistance that transforms lives through compassionate service.
