In this vital role, you will provide essential assistance with daily activities, monitor health conditions, and promote independence for clients in their own homes. Your energetic approach and attention to detail will help create a safe, comfortable, and nurturing environment for those in need of care. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to ensure optimal health status. Support medication reminders. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques. Assist with catheter care. Use equipment such as Hoyer lifts and other assistive devices to safely transfer clients with mobility challenges. Perform light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a hygienic environment. Collaborate with nursing staff and other healthcare professionals to coordinate patient care plans effectively. Follow HIPAA guidelines strictly to protect patient confidentiality at all times. Qualifications Proven experience working in assisted living facilities, nursing homes, or home health environments; long-term care experience is highly valued. Strong knowledge of medical terminology and caregiving best practices for seniors and individuals with disabilities. Experience working with individuals with developmental disabilities or chronic conditions like Alzheimer's disease enhances your suitability for this role. Ability to observe patient conditions closely and communicate effectively about changes or concerns. Demonstrated compassion, patience, and professionalism in providing patient-centered care. Valid driver's license and reliable transportation are often necessary for home visits; flexibility for fill in shifts may be required.
