Job Overview Weekend Aide needed in Newark 12 hours total weekly, Join our compassionate team as a Home Health Aide and make a meaningful difference in the lives of patients requiring personalized care in their homes. In this vital role, you will provide essential support to individuals with diverse health needs, including elderly patients, those with disabilities, and individuals receiving hospice or end-of-life care. Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts. Monitor patient health by observing vital signs, recording medical documentation accurately in Electronic Health Record (EHR) systems, and reporting changes to healthcare professionals promptly. Administer medications following prescribed schedules and ensure proper medication management while maintaining compliance with HIPAA guidelines. Provide specialized care including Alzheimer's care, dementia care, hospice support, and end-of-life assistance to promote comfort and dignity. Support patients with medical procedures such as catheter care, wound care, and basic life support (BLS), including CPR when necessary. Perform light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring a safe environment to uphold home & community care standards. Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met with empathy and professionalism. Qualifications Proven experience working as a Home Health Aide or in assisted living facilities, nursing homes, or long-term care settings. Strong knowledge of medical terminology, patient observation techniques, and home health care regulations. Experience working with individuals with developmental disabilities or geriatrics is highly preferred. Certification in First Aid and CPR is required; additional training in hospice or dementia care is a plus. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts. Familiarity with EMR/EHR systems for medical documentation and client communication. Excellent interpersonal skills with a compassionate approach to caregiving while maintaining patient confidentiality under HIPAA standards.
