Overview Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. This position offers an exciting opportunity to make a meaningful difference in patients' lives by supporting their daily needs and promoting independence. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that enhances well-being and quality of life. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, recording vital signs and changes in condition accurately in electronic health record (EHR) or medical documentation systems Provide companionship and emotional support Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment Use medical terminology appropriately when documenting patient care and communicating with healthcare teams Operate assistive devices like Hoyer lifts safely to transfer patients and prevent injury Qualifications Proven experience in home health care, assisted living, nursing homes, or long-term care environments Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation Knowledge of dementia care, Alzheimer's care, hospice support, and working with individuals with developmental disabilities or disabilities Valid CPR certification Ability to perform patient observation and accurately record vital signs such as blood pressure, pulse, temperature, and respiration Experience with medical procedures including catheter care, medication administration, and hospice care support preferred Strong communication skills to collaborate effectively with healthcare teams while maintaining HIPAA compliance Join us to provide compassionate home health services that truly make a difference!
