Summary :

Responsible for providing quality care to program participants by implementing the plan of care for the individual. Responsible for ensuring adequate staffing levels in the home at all times, which may include coverage for staff call-offs or scheduled time off. The position provides leadership and expertise in customer service, behavior support, support of staff and families involved in day to day interactions.

Essential Duties and Responsibilities include:

Operation of computer, phone and office equipment for business/service, light lifting associated with day to day operations, good communication skills and ability to provide transportation.

• Provides quality care to program participants in an environment that promotes their rights, freedom of choice and individuality.
• Provides training, support, and supervision to HPC staff.
• Responsible for approving staff payroll.
• Responsible for oversight of the medication in the home.
• Maintains schedules of staffing in the home and arranges for staff call-outs, scheduled vacation/wellness, etc.
• Must become knowledgeable about the Individual Service Plan (ISP) and provide care according to the participants' plan.
• Responsible for maintaining the comfort and privacy of the participants in the delivery of care, which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, transferring, ambulation, and assistance with communicating their needs.
• Performing household responsibilities of the program, which may include grocery lists in conjunction with dietary needs, grocery shopping, meal planning, cooking, and cleaning of the home.
• Completes necessary documentation according to the standards required by TWi.
• Assists with processing of Unusual and Major Unusual Incidents .
• Must be able to communicate effectively with participants, colleagues, family members, guardians, case workers, and community members.
• Facilitates working relationships with family members and guardians.
• Brings staffing concerns as deemed necessary to Director of Operations.
• Provides crisis assistance for individuals as may be necessary. Assists in training of individuals.
• Responsible for providing transportation for participants to appointments, errands, and community events.
• May be responsible for development of the ISP in coordination with the SSA.
• May assist with communication with SSA staff for the purpose of coordinating county board programming. Ensures services are being provided in accordance with Individual Service Plan.
• Ensures compliance and quality assurance, when necessary, with all applicable federal, state and local rules, regulations, board policies, and accreditation standards.
• Must review Individual Service Plans and accurately monitor hours of service provided. This may include data entry; type of service provided during the course of the day/night.
• Ability to independently problem solves and offer solutions.
• Ability to remain patient and adapt to ever changing situations that may occur.
• Must comply with all policies and procedures set forth by TWi.
• Must be able to work with a team, but also able to work independently.
• Responsible for being a role model by displaying appropriate decorum, dress, hygiene and attendance.
• Fully understands the rights of participants and promptly reports any evidence of abuse or any suspected violation of a participant rights.
• All other duties as assigned.
