This position offers an exciting opportunity to provide compassionate, personalized care to diverse populations, including seniors, individuals with disabilities, and those recovering from illness or managing chronic conditions. Your expertise will help clients maintain independence, dignity, and comfort while receiving top-tier support tailored to their unique needs. This paid position emphasizes professionalism, empathy, and proactive care in a dynamic environment. Responsibilities Deliver comprehensive patient observation to monitor health status and identify changes promptly Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Administer medications and perform medical procedures such as catheter care under proper protocols Monitor vital signs like blood pressure, pulse, temperature, and respiration; document findings accurately using Electronic Health Record (EHR) systems Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, and hospice care Operate medical equipment including Hoyer lifts and other assistive devices safely and effectively Prepare nutritious meals and assist with meal planning while maintaining food safety standards Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment in compliance with HIPAA regulations Document all patient interactions and medical records thoroughly in accordance with healthcare regulations Collaborate with healthcare teams to ensure continuity of care and update medical documentation regularly Work with individuals with developmental disabilities or disabilities to promote independence and well-being Skills Strong patient observation skills to detect subtle health changes quickly Experience working in assisted living facilities or nursing homes is highly desirable Proficiency with EMR (Electronic Medical Records) or EHR systems for accurate documentation Knowledge of medical terminology relevant to home health care settings Ability to assist with ADLs efficiently while respecting client dignity and preferences Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice or dementia training preferred Familiarity with hospice care protocols, catheter care procedures, and medication administration processes Experience working with individuals with disabilities or developmental challenges is a plus Competence in using mobility aids like Hoyer lifts safely and effectively Ability to perform patient care tasks such as vital signs monitoring and medication management accurately Excellent communication skills for effective interaction with clients, families, and healthcare teams Knowledge of HIPAA regulations to ensure confidentiality of patient information Flexibility to work live-in shifts or part-time schedules depending on client needs Embark on a rewarding career where your compassion makes a real difference!
