This position offers the opportunity to work closely with healthcare professionals and contribute to quality home health care that truly matters. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or transfer belts Monitor and record vital signs like blood pressure, pulse, temperature, and respiration to track health status accurately Support medication administration in accordance with healthcare plans and document all medical records securely within Electronic Health Record (EHR) systems Provide specialized care such as Alzheimer's and dementia care, memory care, hospice support, catheter care, and working with individuals with developmental disabilities or disabilities Observe patient conditions closely, report changes promptly to healthcare providers, and maintain detailed medical documentation following HIPAA guidelines Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients Utilize medical terminology confidently when documenting care activities and communicating with healthcare teams Requirements Proven experience in home health care or long-term care environments such as nursing homes or assisted living facilities Strong knowledge of patient observation techniques and medical procedures including CPR certification and vital signs monitoring Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation of patient information Ability to assist with complex caregiving tasks such as medication administration, catheter care, hospice support, and dementia-specific needs Excellent communication skills to work effectively with patients, families, and healthcare professionals Valid certification in CPR; additional certifications like Hoyer lift training or dementia care are a plus Experience working with individuals with disabilities or in community-based home care settings is highly desirable Join us in delivering heartfelt care that empowers individuals to live safely and comfortably at home.
