Duties Assist residents with Activities of Daily Living (ADLs), including personal hygiene, grooming, dressing, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor patients' vital signs, observe health status, and report changes to healthcare professionals to ensure optimal care. Prepare nutritious meals and assist with meal planning in accordance with dietary needs and care plans. Support medication administration following HIPAA regulations and established protocols to ensure safety and compliance. Provide patient care during home health visits or in community settings, including Alzheimer's care, dementia care, and memory support for individuals with neurodevelopmental disorders or developmental disabilities. Maintain detailed documentation of patient progress, care plans, and incident reports while ensuring confidentiality and adherence to HIPAA standards. Facilitate transportation for appointments or community activities, providing safe personal transportation assistance as needed. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Demonstrated ability to perform patient observation, monitor vital signs, and assist with medication administration. Knowledge of specialized care such as Alzheimer's disease, dementia care, hospice care, or autism support enhances your effectiveness in this role. Experience working with individuals with disabilities—including intellectual disabilities or neurodevelopmental disorders—will be highly valued. Familiarity with patient transfer methods like Hoyer lifts and patient personal care assistance ensures safe handling practices. Strong communication skills for client interaction and team collaboration are essential for success. Certification in CPR and First Aid is required; ongoing training in manual handling and heavy lifting techniques is beneficial.
