Job Description
Job Summary Join our compassionate team as a Home Health Aide, where your dedication and caregiving skills will make a meaningful difference in the lives of patients and their families. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient health status, including vital signs such as blood pressure, pulse, respiration, and temperature Support medication administration following proper protocols and documentation procedures Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately record patient information and care activities Provide specialized care for patients with conditions such as dementia, Alzheimer's disease, or other memory impairments, including memory care strategies Assist with medical procedures such as catheter care, Hoyer lift transfers, and hospice support when needed Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry and meal preparation Document medical records accurately while adhering to HIPAA privacy regulations Support patients during emergencies or special needs situations by following established protocols Requirements Proven experience in home health care or long-term care environments such as nursing homes or assisted living facilities Knowledge of medical terminology and caregiving best practices for seniors and individuals with disabilities Valid CPR certification and training in emergency response procedures Experience working with people with developmental disabilities or dementia-related conditions is highly desirable Ability to operate EMR/EHR systems efficiently for documentation purposes Strong patient observation skills to identify changes in health status promptly Compassionate attitude with excellent communication skills to interact effectively with patients and their families Ability to lift or transfer patients safely using equipment like Hoyer lifts when necessary Flexibility to work live-in shifts or part-time schedules based on patient needs Join us if you're passionate about providing empathetic, professional home health care that truly improves lives.
