Job Summary Join our compassionate team as a Home Health Aide & Caregiver, where your dedication will make a meaningful difference in the lives of individuals requiring personalized care and support. In this vital role, you will provide high-quality assistance with daily living activities, monitor patient health, and ensure a safe, comfortable environment for clients across various care settings. Your energetic approach and attention to detail will help promote independence, dignity, and well-being for those in your care. This paid position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or palliative care. Duties Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare professionals. Administer medications accurately following medical documentation protocols and home healthcare regulations. Support patients with medical needs such as catheter care, wound care, and managing medical devices while maintaining strict patient confidentiality in accordance with HIPAA standards. Provide companionship and emotional support to enhance mental well-being, especially for clients with dementia or Alzheimer's disease. Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment conducive to health and comfort. Document all patient interactions and care activities in electronic health record (EHR) or electronic medical record (EMR) systems accurately and efficiently. Follow safety procedures during patient transfers using proper techniques to prevent injury for both clients and caregivers. Support end-of-life or hospice care needs with compassion, patience, and respect for the client's dignity. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred. Demonstrated knowledge of medical terminology and basic life support (BLS), including CPR certification. Experience working with individuals with developmental disabilities or geriatrics is advantageous. Familiarity with home health care assistance protocols, home & community care practices, and hospice or palliative care services. Proven ability to work independently while maintaining excellent client communication skills. Experience operating EMR/EHR systems for accurate medical documentation and record keeping. Physical ability to perform heavy lifting when necessary and assist with patient transfers safely using appropriate equipment such as Hoyer lifts. Must have some weekend availability.
