Job Description

Overview Must be licensed and/or experienced with total care patients including feeding, bathing, toileting, and mobility. Apply today to learn more. Ideal applicants will be passionate about senior care and thrive in diverse home care settings with daytime and weekend availability with reliable transportation. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and transfer devices Monitor patient vital signs such as blood pressure, pulse, temperature, and respiration; accurately document medical information using Electronic Health Record (EHR) systems Medication reminders following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality Provide specialized care for individuals with dementia, Alzheimer's disease, or other cognitive impairments through memory care techniques and patience-driven approaches Support patients with catheter care, wound management, hospice needs, and other complex medical procedures under supervision or as trained Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment tailored to each patient's needs Document all patient interactions thoroughly in medical records and communicate effectively with healthcare teams regarding changes in condition or concerns Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred Demonstrated ability to observe patient conditions closely and respond promptly to changes in health status Familiarity with EMR/EHR systems for accurate documentation of patient care activities Hands-on experience assisting with ADLs, medication administration, and medical procedures such as catheter care or vital sign monitoring Knowledge of medical terminology relevant to long-term care, dementia, hospice, and complex medical conditions Certification in CPR is required; additional certifications such as hospice care training or dementia care are a plus Experience working with individuals with developmental disabilities or disabilities is highly valued Join us today to be part of a passionate team dedicated to delivering compassionate home health services!
