Responsibilities Assist clients with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor patients' vital signs, observe changes in health status, and report findings accurately while adhering to HIPAA regulations for confidentiality. Prepare nutritious meals aligned with dietary plans and assist with meal consumption to ensure proper nutrition. Support medication administration following prescribed care plans and ensure proper documentation of all patient care activities. Provide companionship and emotional support, fostering a respectful and engaging environment that promotes mental well-being. Maintain a clean and safe living environment by performing light cleaning duties and ensuring safety protocols are followed at all times. Assist with transportation needs for appointments or community outings, ensuring client safety during transfers and travel. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient observation techniques, medication administration, vital signs monitoring, and first aid/CPR certification. Experience supporting individuals with Alzheimer's disease, dementia, memory care needs, autism spectrum disorder, or other neurodevelopmental disabilities is advantageous. Ability to perform heavy lifting safely and use patient transfer equipment such as Hoyer lifts. Strong communication skills for effective client interaction and documentation in compliance with HIPAA standards. Familiarity with care plans, medication management, patient personal care assistance, and assisting with ADLs. Valid certifications in first aid and CPR; additional training in hospice care or geriatrics is a plus.
