Job Description

Job Overview Join our dedicated healthcare team as a Home Health Aide (HHA), where you will play a vital role in providing compassionate, personalized care to individuals in their homes. This rewarding position offers the opportunity to make a meaningful difference in patients' lives by assisting with daily activities, monitoring health status, and ensuring comfort and safety. As an HHA, you will work closely with clients, families, and healthcare professionals to deliver high-quality home health services that promote independence and well-being. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient conditions, including vital signs and overall health status Help with medication reminders and administration under supervision, ensuring compliance with medical instructions Support patients with mobility aids like Hoyer lifts and assist with transfers to prevent falls or injuries Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment Document patient care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while adhering to HIPAA regulations Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions; including memory care techniques and behavioral support Assist with medical procedures such as catheter care and hospice support as directed by healthcare professionals Support patients during emergencies or changes in condition by communicating effectively with family members and emergency responders Qualifications Prior experience in home health care, nursing homes, assisted living facilities, or long-term care environments is highly preferred Strong knowledge of medical terminology, vital signs measurement, medication administration, and documentation practices Experience working with individuals with developmental disabilities or disabilities requiring specialized care such as Alzheimer's or dementia care Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications like first aid are a plus Ability to operate EMR/EHR systems efficiently for accurate recordkeeping Excellent patient observation skills combined with compassionate caregiving abilities Physical stamina to assist with mobility tasks including the use of lifts and transfers; ability to lift safely as needed Knowledge of HIPAA regulations to ensure patient confidentiality at all times Valid driver's license and reliable transportation are often necessary for home visits Join us in delivering exceptional home health services that empower individuals to live safely and comfortably at home. $16.00 - $18.00 per hour
