Job Description

Haven Home Health & Hospice is seeking a compassionate and dependable Full-Time Hospice Aide (CHHA) to join our growing team. As a Hospice Aide, you will play a vital role in supporting patients and their families by providing personal care, comfort, and dignity during their end-of-life journey. This position offers the opportunity to make a meaningful difference while working alongside an experienced and supportive hospice team. Responsibilities Provide personal care and assistance with activities of daily living, including bathing, grooming, dressing, and toileting. Assist with mobility, transfers, and ambulation. Help maintain a safe and comfortable environment for patients. Assist with meal preparation and feeding as directed. Observe and report changes in patient condition to the clinical team. Obtain and document vital signs, intake/output, and other patient information as directed. Perform light housekeeping tasks related to patient care. Complete accurate and timely documentation. Communicate effectively with patients, families, and members of the hospice team. Qualifications Current Oklahoma Certified Home Health Aide (CHHA) certification required. Previous hospice, home health, CNA, or direct patient care experience preferred. Compassionate, patient-focused approach to care. Strong communication and interpersonal skills. Valid driver's license, reliable transportation, and current automobile insurance. Ability to travel throughout the service area. Ability to work independently while remaining part of a collaborative team.
