Provide quality care and assistance to individuals with developmental disabilities attending the day program setting to maximize each individual's potential
Responsibilities:
Provide quality care to consumers in an environment which promotes their rights, dignity, freedom of choice, and their individuality as illustrated by the following: Provide individualized attention, which encourages each consumer's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being. Ensure active engagement in constructive age appropriate activities. Knowledgeable of the individualized care plan for each consumer and provide support to the consumer according to the care plan. Contribute to the care planning process by providing the appropriate personnel with specific information and observations of the consumers' needs and preferences. Attend to the individual needs of the consumers, which may include assistance with personal hygiene, feeding, incontinent care, toileting, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements. Maintain the comfort, privacy, and dignity of each consumer in the delivery of services to them. Interact with consumers in a manner that displays warmth and promotes a caring environment. Fully understand all aspects of consumers' rights, including the right to be free of restraints and free from abuse. Responsible for reporting to the supervisor and/or appropriate personnel any incidents or evidence of consumer abuse or violation of consumers' rights. Assist in maintaining a safe, neat and clean environment; report environmental deficiencies to the supervisor, such as lighting or equipment problems. Protect the personal belongings of each consumer, including furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to the established policy and participate in efforts to locate missing items. Observe consumers for changes in condition or behavior and promptly report these changes to appropriate personnel. Provide care that maintains each consumers' skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent consumers, turning, repositioning immobile consumers, and applying moisturizers to fragile skin, etc. Assist with orienting consumers and their families to the program upon admission and/or transfer. Lift, move, and transport consumers using proper body mechanics or lifting devices for accident prevention. Communicate and interact effectively and tactfully with the consumer, visitors, families, peers, supervisor, and other personnel. Assist and escort consumers for appointments as needed. Participate in activities and functions as directed Practice careful, efficient, and no-wasteful use of supplies. Complete documentation of care provided or other information in keeping with program policies. Perform all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves, and proper disposal of soiled materials. Administering of Medications utilizing the proper techniques as trained in the Pennsylvania Department of Public Welfare Medication Administration Training Program. Evaluates consumers annually to write Assessment, Individualized Program Plans/ISP, etc Coordinate IPP/ISP meetings. Write and distribute 65 day reviews. Attend team meetings as required. Maintain written monthly reports of progress on program plan goals. Direct the Activity Assistant(s) assigned to the room, including orientating and providing on-going training in the implementation of programs, goals, activities, etc. Provides necessary information to assist the Associate Director or Director with the completion of Activity Assistant(s) evaluations. Develop daily schedules for both morning and afternoon activities. Devise and implement behavior management plans, data sheets, and present reports to the Human Rights Committee as required. Ensure that verbal and written communication is provided daily to parents or residential staff as necessary or as requested. Plan community outings for individuals and seek volunteer opportunities for the individuals to participate in. Assist the Associate Director with various administrative tasks to support program operations as directed; may also be assigned special projects.
