Job Description

Medical Social Worker (MSW) - Home Health - Per Visit / PRN Penn Home Health Care is seeking a compassionate Medical Social Worker (MSW) to support patients and families receiving home health services.
Service Areas:
Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, PA Position Type:
Per Visit /
PRN Responsibilities:
Conduct psychosocial assessments. Identify community resources and support services. Assist patients and families with long-term planning and coping strategies. Coordinate referrals and community services. Participate in interdisciplinary care planning. Complete required documentation.
Qualifications:
Master's Degree in Social Work from an accredited program. Current Pennsylvania licensure if required by applicable regulations. Experience in healthcare, home health, hospice, or related setting preferred. Valid driver's license and reliable transportation.
