Job Description

Essential Responsibilities:
1. Provide age and functionality appropriate program activities that include assistance, training, and support for the acquisition of functional skills, personal needs, communication, and personal adjustment. 2. Assist in the development of goal plans and implement as directed. 3. Direct communication of important information to supervisory staff. 4. Correctly and accurately record an individual's activities and progress on case notes, goal plans, behavior plans, community outing sheets, and other forms as required. 5. Provide Individuals with opportunities for community integration. 1. Report medical observations and concerns to the Manager, Assistant Manager, and / or Lead Staff. 2. Document seizures, incidents, etc. on appropriate forms. 3. After becoming trained in medication administration, administer medications to individuals as prescribed. 4. Encourage individuals to follow doctor's recommendations regarding healthy choices and wellness. Personal Care 1. Assist individuals with personal care and supervision, as outlined in their Individual Support Plan, which may include, but not limited to: a. Bathing b. Toileting c. Grooming d. Dressing e. Eating meals 1. Transport Individuals to and from community outings using Jessica & Friends Community and / or personal vehicle. 2. Supervise general group activities for the Individuals. 3. Handle emergencies based on organizational protocol. 4. Communicate with supervisor and other staff through appropriate means. 5. Maintain the general upkeep of the program space. 6. Assist with the maintenance of Jessica & Friends Community vehicles. 7. Attend staff meetings, trainings, and in-services as scheduled and / or review / read the minutes of those meetings. 8. Participate in twenty-four hours of training annually, as required in the regulations. 9. Maintain excellent attendance of self in keeping with the Attendance Policy. 10. All other duties as assigned.
Job Requirements:
Must be at least 18 years of age with a high school diploma or GED. At least two years of human services experience can be substituted for the high school diploma or GED. Additional education and / or experience preferred. Must also have a valid driver's license, valid auto insurance, and valid car registration. Will primarily use Jessica & Friends Community vehicles to transport individuals to and from activities, appointments, and workshops; however, it may be necessary to use personal vehicle. Must have at least two years of driving experience to transport individuals to and from activities, appointments, and workshops, using Jessica & Friends Community and / or personal vehicle. Must submit to a criminal background check, a driver's record check, an initial physical, and substance screening. This is a safety sensitive position.

Work Schedule:
Both full-time and part-time positions with various hours usually Monday through Friday between the hours of 7:30 a.m. and 4:00 p.m.
