Job Responsibilities:

Ensure health and safety of individuals Provide emergency support when necessary Participate as a team member Assist individuals with supporting their goals Assist with daily living skills such as personal hygiene care, shopping, laundry and meal preparation Transportation to doctor appointments, other programs, employment, etc. Documentation on online system required Ability to read and implement and Individual Support Plan (ISP)

Job Requirements:

Driver's License, Social Security Card and High School / GED diploma for pre-employment review Operational Vehicle Vehicle Registration Vehicle Insurance Pre-Employment Criminal Background, Physical Examination, TB Test, Drug-Screen Ability to be in the group home for 24 hours
