This role offers an opportunity to make a meaningful difference in the lives of residents by delivering personalized support in client home. Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary. Monitor residents' health status by observing vital signs, patient behavior, and overall well-being; promptly report any concerns to healthcare professionals. medications reminders following HIPAA regulations and care plans while ensuring accurate documentation. Prepare nutritious meals in accordance with dietary restrictions and preferences; maintain cleanliness during food preparation and dining areas. Support residents in participating in community activities, social outings, and recreational programs to promote engagement and independence. Maintain a safe environment by adhering to infection control protocols, performing regular cleaning duties, and managing safety hazards. Follow individualized care plans tailored to each resident's needs. Document care activities accurately while respecting residents' privacy and confidentiality at all times. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care settings is highly desirable. Familiarity with working with individuals with disabilities— dementia, or memory care needs—is essential Strong interpersonal skills with the ability to communicate effectively with residents, families, and healthcare teams. Basic cleaning skills to maintain hygiene standards within the resident's environment. Valid certifications in CPR and first aid; additional training in hospice or home health care is a plus. Commitment to maintaining HIPAA compliance and respecting resident confidentiality at all times.
